Green Bay Invitational

Pre-Tournament - Please collect the following:

• A "Permission to Travel Form" from your State Association (OUT OF STATE TEAMS ONLY - Not needed from Wisconsin teams).
   
• Current US Youth Soccer Player Pass Cards for ALL PARTICIPANTS U11 and up (no cards for U8 and U10)
   
  • Player pass cards come from your state association/district or club.
  • Coaches must also provide credentials.
     
• TWO (2) copies of your current US Youth Soccer 'approved' Team Roster - one (1) copy will be kept by the tournament for the duration of the event.
   
  • Team rosters come from your state association/district or club, and must be authorized by your USYSA District Registrar
  • All rosters must list the jersey number of each player.
  • ONE (1) copy of your roster MUST BE LEFT with tournament officials at check-in.
     
• A US Youth Soccer Guest Player Form, if bringing guest players
   
  • Players from your club are 'Club Passing' and are not considered Guest Players.
  • Maximum number of allowed 'Club Pass' players - 3.
  • Players from a different club are 'Guest Players'.  Download a Guest Player Form from WYSA (or YOUR STATE ASSOCIATION) website.
  • Number of allowed 'Guest' Players: 3.
     
• Signed WYSA Release of Liability and Consent for Medical Treatment forms for ALL PARTICIPANTS
   
  • Make sure you only use the current form.
  • Copies for all players will be checked at registration.
     
• Review Tournament Rules and Field Locations
   
• Download documents, print for team as necessary.

At the tournament:

• Check in your team!
   
  • Check-in times are: Friday (3:00 pm - 7:00 pm) and Saturday (7:00 -11:00 am).
  • Check in at the registration tent at Akzo Nobel Sports Complex.
  • You must have your team checked-in at least 1 hour prior to your first game or during the times listed above.

Documents to bring to check-in:

• Your State Association approved "Application to Travel" form (for NON-WISCONSIN Teams only.)
• Two Copies of US Youth Soccer approved Team Roster (& Guest player form if applicable.)
• Current US Youth Soccer player pass cards for all players.
• WYSA Release of Liability and Consent for Medical Treatment forms for all players.
• Emergency contact information for manager and/or coach while attending the tournament.
