To ensure you are guaranteed your correct size and to save your time standing in line the day of the event please complete this order form and mail to Quali T, Inc., along with payment, by May 31st (no exceptions). in you must provide your credit card information for payment at the bottom of this order form.

All pre-orders received by May 31st will be available for pick up at the Akzo Nobel Park apparel/volunteer tent on June 12th through 14th.

If you are unable to pre-order before the event, Quali T will be onsite Saturday, June 13th and Sunday, June 14th selling the apparel while supplies last! Please note the items sold at the tournament will be priced higher so we encourage you to order through the pre-sale for the best cost! Questions,
